Bariatric surgery is a recommended option for patients who face severe or moderate obesity, provided they meet specific criteria.

Who is a candidate for surgery?

High BMI: Patients with a Body Mass Index (BMI) over 40 kg/m², regardless of the presence of other health conditions.

Moderate BMI with Comorbidities: Individuals with a BMI between 35 and 40 kg/m², provided they have comorbidities.

Age Range: Surgery is recommended for adults between 18 and 65 years old, following individual assessment. However, in exceptional cases, it may be considered for patients over 65 years of age, or even between 16 and 18 years, with parental consent and the recommendation of two different doctors.

Indication for Metabolic Syndrome

Additionally, bariatric surgery is also an option for patients diagnosed with Metabolic Syndrome, a condition characterized by metabolic disorders that contribute to obesity and other serious diseases such as hypertension, dyslipidemia, diabetes, and cardiovascular diseases. In these cases, surgery is considered when:

  • Clinical treatment does not produce satisfactory results.
  • The patient has a BMI between 30 and 35 kg/m².
  • The patient’s age is between 30 and 70 years.
  • The diagnosis of diabetes was made less than ten years ago.

The relationship between obesity and the accumulation of visceral fat is directly associated with Metabolic Syndrome, triggering inflammatory processes that increase the risk of cardiovascular diseases.

When is surgery not recommended?

However, there are cases where bariatric surgery is not indicated, such as:

Low BMI: For patients with a BMI below 35 kg/m².

Specific Conditions: Such as patients under 16 years of age, those with severe genetic diseases like Prader-Willi Syndrome, uncontrolled psychiatric disorders, substance abuse, severe cardiopulmonary diseases, portal hypertension with esophageal-gastric varices, untreated Cushing’s syndrome, endocrine tumors, and patients unable to adhere to postoperative dietary guidelines for various reasons.

The decision regarding the feasibility of bariatric surgery should be carefully assessed by both the patient and the physician, taking into account all individual aspects of health and lifestyle.
